Free location risk assessment templates

Blank Word files for the consultation and the record of a location assessment (often called a location risk assessment), for a children's home or supported accommodation. No email asked, and nothing in them but placeholders in square brackets. The location assessment evidence pack fills in the public data for your area.

What a location risk assessment covers

Ofsted sets no format, but a review that stands up to an inspector usually holds five things. Each line names the template or the part of the pack that holds it.

  • The area's public data. Recorded crime and its trend, deprivation, what children's services record about exploitation and going missing, the roads, and the schools, transport and green space nearby. The templates leave this to you; the evidence pack gathers it chapter by chapter.
  • Consultation with the police and the host authority. The consultation letters, each with a reminder, and the consultation tracker for who was asked, when and what came back, including no reply.
  • The risks and opportunities, with controls. The risk matrix: each hazard scored for severity and likelihood, its controls, and the risk that is left, with an owner and a date.
  • The children's or young people's views. The views prompt sheet, used on paper, and a table for what changed as a result.
  • The review record and sign-off. The review record: the calendar year covered, why the review was held, what it concluded and who signed it off, by role.

Using them

  • Replace every part in square brackets. Keep the completed files confidential: they record where children live.
  • Record roles and bodies, never anything about any child or young person. What a child tells you belongs in their own records.
  • The templates hold the structure. The public data, the letters filled in for your area and the review record kept for the year come with the location assessment evidence pack, priced below.

Read what regulation 46 asks and what regulation 6 asks of supported accommodation.
